Informed Consent & Client Agreement
This Agreement is intended to provide the undersigned client (hereafter referred to as the “Client”) with important information regarding the nature, scope, and limitations of Brainwave Training / Neurofeedback services provided by Dena McComb (hereafter referred to as the “Provider”).
Any questions regarding this Agreement should be discussed prior to signing.
Nature of Brainwave Training / Neurofeedback
Brainwave Training, also known as Neurofeedback, is a non-invasive, educational, and self-regulation–based process designed to support awareness and regulation of brain activity.
This training is sometimes used by individuals seeking support with focus, stress regulation, sleep, pain perception, emotional reactivity, or overall nervous system balance. Responses to training vary from person to person.
Brainwave Training does not constitute medical treatment, psychotherapy, or diagnosis.
Scope of Practice
The Provider:

Is not a physician, psychologist, or licensed mental health provider

Does not diagnose or treat medical or psychiatric conditions

Does not prescribe or modify medications
-
Does not claim to cure or treat any condition
The Provider offers Brainwave Training as an educational and supportive modality, intended to complement — not replace — appropriate medical or mental health care.
Research & Individual Response
While neurofeedback has been studied and used for a variety of purposes, research is ongoing, and mechanisms of action are still being explored.
The Client understands that:

Results are not guaranteed

Some individuals may experience noticeable benefit, while others may not
-
Any improvements may diminish after training ends

Periodic follow-up or booster sessions may be helpful for some individuals
Risks & Client Responsibility
Brainwave Training is considered low risk and non-invasive. However, as with any self-regulation or nervous-system-based practice, individuals may experience temporary or unexpected responses, including but not limited to fatigue, changes in mood, headache, or changes in sleep.
The Client agrees that:

Participation is voluntary

They are responsible for monitoring their own responses

They may pause or discontinue training at any time

They will consult a qualified healthcare provider regarding medical or mental health concerns
The Client agrees to continue any current medical or therapeutic care unless otherwise advised by a licensed healthcare professional.
Assumption of Risk & Release of Liability
By signing this Agreement, the Client knowingly and voluntarily assumes all risks associated with participation in Brainwave Training.
The Client hereby WAIVES, RELEASES, AND HOLDS HARMLESS the Provider from any and all claims, demands, or causes of action arising from participation in Brainwave Training, including but not limited to lack of improvement, temporary discomfort, or perceived worsening of symptoms, to the fullest extent permitted by law.
Communication With Healthcare Providers
If coordination with a primary care provider or other healthcare professional is requested or deemed appropriate, the Client understands that a separate written Release of Information will be required prior to any communication.

By signing below, the Client acknowledges that:

They understand this service is not medical or mental health treatment

Participation is voluntary

They accept responsibility for their own well-being
